• Passport size photo to be uploaded in jpg format
• Signature scanned with white back ground in jpg format
• Plus two certificate copy in pdf format
• Aadhar card copy

DOCUMENTS TO BE SUBMITTED ON ADMISSION – BSc MLT

Original and 2 copies of the following certificates should be submitted along with the Application form, failing the same lead to the rejection of application.

1. SSLC or equivalent Certificate to prove date of birth
2. Aadhaar card
3. Mark list(s) of qualifying Examination(HSC/equivalent)
4. Conduct certificate
5. Transfer certificate
6. Migration certificate (If necessary)
7. Community/Caste certificate
8. Nativity certificate
9. *A medical fitness certificate showing suitability to undergo the course
   * (A prescribed format should be collected from the office at the time of admission)
